You can access your account at club4fitness.clubautomation.com . Once you are on the main page you scroll down and you will find ( Access my Account) it will ask for your email to reset your password .

- Freeze requests must be received more than 5-days prior to billing date. All recurring payments already scheduled will be processed within the 5-day notification period.
- Primary account holders can freeze any add-ons associated with their account.
- Add-ons can freeze their account only.
- The total amount of time permitted to freeze an account is 3-months per calendar year. You are only allowed one freeze at a time*
- There is a fee for each month you request to freeze for; $10 for individuals, $20 for a couple, $25 for a family membership. There will be no additional rejoining fees to begin using your membership once your freeze is removed. You are responsible for payment of the remainder of your prorated membership dues.

*If you would like to extend your freeze, you must contact us at least 5-days prior to your reinstatement date
- A 30-day written notice is required to cancel any membership(s) and recurring services.
- All recurring payments already scheduled will be processed within the 30-day notification period.
- Primary account holders can cancel any add-ons associated with their account.
- Add-ons can cancel their account only.
- There are no refunds on collected membership dues or pre-paid services.
- These expire with the cancellation of your membership. Cancellation of annual memberships may be subject to early termination fees.

Late or Returned Item Charges. A Late Fee of $35 will be assessed for returned checks, insufficient
funds, closed accounts, frozen or declined credit cards or similar circumstances, which result in late
or delayed payment to The Club For Fitness. Member is responsible for providing accurate and
updated information on their EFT account to ensure timely receipt of payment. The Club For Fitness reserves the right to the re-attempt collection of Member’s outstanding balance until such time as
Member’s account is current.
